Amid providing care for chronic diseases like diabetes and hypertension, our providers treat acute problems such as coughs, colds, and the flu. As a patient centered medical home, we strive to incorporate you into your care management. You can expect comprehensive, coordinated, team based care from our providers and staff. We hold ourselves to the highest level of commitment to your well-being and expect you to do the same. For this reason, we provide a number of preventative care services such as vaccine administration, annual wellness visits, and colorectal cancer screening.

We are excited to offer Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services for our Medicare and Medicare Advantage patients living with diabetes, high blood pressure, heart disease, lung disease, arthritis, and other long-term health conditions. These services provide you with a personal care coordinator that will assist you with medication refills, scheduling appointments, and coordinate other aspects of your care like transportation, community resources and medical equipment. Additionally, our RPM service provides you with a blood pressure monitor that connects directly to our systems via the cloud which allows us to seamlessly monitor your vitals from the comfort of your home.
